Thanks to Jan Luehe and Steve Essery for your help and time. This is the solution to my problem. Glassfish doesn't accept HTML files for the file, only HTM files, so the final line is:

<property name="send-error_1" value="code=404 path=errors/404.htm reason=MY-404-REASON"/>
