It seems GlassFish is unable create new connections with Postgres server.
can you post your
1) persistence.xml
2) connection pool configuration from domain.xml
3) you can set "FINEST" log level for "connector" and "resource-adapter" module log levels and post the log.
4) Is connection validation (preferably, table based validation) enabled ?
