Did you set jvm by doing -
$GLASSFISH_HOME/bin/asadmin create-jvm-options -Dcom.sun.enterprise.web.connector.enableJK=8009?
Can you send your domain.xml? I'd check if mod_jk is enabled correctly since the logging info is missing as you mentioned.
