Could you please use telnet (telnet lvov1.ips 8173) and send the following lines to your server (you must hit return twice at the end)?
GET / HTTP/1.1
Host: lvov1.ips
Connnection: Close
Now have another look in your access logs. Any change now?
BTW: V39 is not correctly configured and thus it is normal that it is deployed to all virtuals servers. If no virtual-servers attribute is given in the application-ref-element this module/app is deployed to all virtual servers. So V39 at least works as expected.
