I am not sure about this use-case.
An instance/cluster should not be marked unhealthy if one application is not correctly deployed. There may be other applications which will be affected if I configure health-checker based on one application.
However you can configure application specific url in load-balancer xml for health checking purpose. Refer to document

It considers response codes between 100 to 500 as instance being healthy and responding to requests.
