In your packaged archive, do you bundle glassfish-resources.xml ?
Can you post your application so that we can see what is going on ?
It looks to me like the application has persistence.xml and it (jndi-name
element) is configured to use an application scoped jdbc-resource (starting
with "java:app/").
[It is possible that Netbeans is generating the
resources/glassfish-resources.xml behind the scenes]
