No, if a entry is already present in the table you obtain the index of
the entry and encode that index. If an entry is not present in the table
you add the entry to the table and encode that entry.

Yes. There are separate tables for distinct information.

I recommend you read Annex D of the specification [1], it contains an
example that should help.
