Customizing the Predefined ORDM Models for MeddWebb

This first customization scenario consists of the following steps:

1. Define a new lookup in the Logical Data Model.
2. Decide on a TYPE_CODE value corresponding to the lookup.
3. Define a new view, MDX_PRESCRIBER_TYP, for the lookup.
4. Insert a set of records into the DWL_CODE_MASTER table, corresponding to the possible sets of value for the lookup with the fixed TYPE_CODE value, as decided as part of step 2.
5. Add a column into a new DWR_PRESCRIBER table, where this new lookup needs to be used.
