Yes, there should be a message saying that a timer failed N redeliveries. But I also saw a problem when a timer fires (it should be fixed in the latest 3.1 builds) during server shutdown, and because the timeout fails to be delivered (for the reasons of the application being stopped), the corresponding timer was removed. Can it be the case? Or do you see that on server restart some timers are restarted as some are not?

If you could have a reproducible test case, I can investigate further...
