Sorry for my late reply, but I was away all of last week.
After careful analyzing all http requests / responses, I found out that the mistake was on my side. I used .shtml for the include file and .html for the main calling file.
Of course this is wrong, and should be the way around, i.e. use .shtml for the main calling file. It now works fine.
