The problem is back again... It seam that it happens not always, but sometimes...
I got the exception again, stopped GlassFish, restarted MySQL, started GlassFish and now I have no failure again. But how long?!
Versions:
* Glassfish: Sun Java System Application Server 9.1_02 (build b04-fcs)
* MySQL: 5.0.67-community-nt
* JDBC connector: mysql-connector-java-5.0.8-bin.jar
