The output from the tool should have included a stack trace from each place that JAR was opened from but for which a close was never done. Please post that stack trace here and we'll take a look. Make sure that you have followed the instructions in the blog entry describing the tool, specifically that you
1. restart the server
2. start the tool
3. deploy the app and use it (if using it is part of the scenario which locks the file)
4. undeploy the app
5. use the tool's "show" command
