About Q2 I think there is some misunderstanding.
lb sends a bad gateway in normal scenarios as well. This means one request which hits an unhealthy instance is dishonored and not sent to the second instance.
It would be great to have an lb configurable to response codes, is this feature planned in the roadmap ?
