How do you set up your concurrent users?
Do you create threads yourself?
Do you use the same EM for all concurrent users?
Can you try a different JPA provider with GlassFish to check if it's the provider or the server that throws the exception (also, look in the server log if there any other exceptions that can point to the cause of the rollback)?
