Hi:

If I understand your first question right, you should get an answer to it from:
https://glassfish.dev.java.net/nonav/javaee5/docs/SJSASEEHAAG.pdf
See Chapters on "Configuring Webservers for Load Balancing" and "Configuring HTTP Load balancing".
See section on "Procedure to Setup Load Balancing in Chapter "Configuring HTTP Load balancing".

Regarding the question on "device host and "device port":
- One can create multiple load-balancer's (configurations) that are associated with different web server's "device host" and "device port". This can be done using the asadmin create-http-lb command or the Admin GUI.
- This provides you the flexibility of using different load balancer configurations with different physical web servers. Which means, if required, you can load-balance requests received on different web servers in different ways (to different clusters / listeners for example).

For more information on this see section on "Using Apache web server" in chapter "Configuring Webservers for Load Balancing"

thanks.

varun.
