I checked with Jamey Wood and here's the situation regarding attachments:
The described behavior happens when your session has timed out. You can
tell this has happened when the 'Login in' link shows up in the
upper-right hand area of the wiki page (by the search box). If you go
ahead and login again, you should be able to use the attachments tab.
A problem does exist where sessions are being timed out too quickly, but
Jamey is trying some new settings that should help with this.
