Quick update.
I was able to get our application running on Glassfish 2.1.1 and the problem is gone.
This appears to be a quite serious bug in v3.
We will be migrating to 2.1.1 instead of v3 because of this bug. I general v3 just doesn't seem quite production ready. Maybe 3.1 will be.
