Yes!! Good catch! Sorry I didn't see it before. Passivation of the stateful session bean did result in an exception:

There was no exception trace in the log. Is that a small bug in the container code?
I will fix this today along with working on an unrelated database locking bug that I'm trying to address. By tonight, I should have a fix for the session bean passivation problem. I'll have to watch it for awhile to see if I continue to see exceptions from this problem because I don't know how to reproduce this on demand.
Thanks for your continued help on this. I'll report back as soon as I have more information.
