This may be related to bug:
https://glassfish.dev.java.net/issues/show_bug.cgi?id=14247
At some point _register_instance started returning a Warning in some situations so
create-instance doesn't create the instance filesystem in that case.
create-instance needs to be changed to create the filesystem on SUCCESS or WARNING.
I'm testing the fix now.
